Onlay - resin-based composite - two surfacesEndodontic therapy - molar tooth (excluding final restoration). The fee for palliative treatment is Disallowed when done In Conjunction With root canal therapy by the same dentist/dental office on the same date of service. Palliative treatment is payable on a separate date of service for relief of pain. Incompletely filled root canals are not payable, and the fee for the endodontic therapy is Disallowed. Post removal is not included in this procedure.
